Abstract
Third molars, or “wisdom teeth,” are a frequent source of dental complications in modern humans, so much so that clinically removing them has become a rite of passage. However, third molar agenesis—their congenital absence—appears to be on the rise. Studies have linked this pattern to potential genetic and environmental factors, yet explanations remain fragmented and insufficiently integrated. This meta-analysis synthesizes global evidence on third molar agenesis through an explicitly evolutionary lens, aiming to situate contemporary variation within long-term patterns of human dental reduction. Findings indicate that agenesis was likely rare in ancestral populations but now varies significantly across ethnic and geographic groups, with particularly high frequencies observed in Asian populations. These patterns reflect not a single adaptive shift, but rather an ongoing evolutionary process shaped by the interaction of genetic architecture, developmental constraints, and changing environments, including those associated with industrialization and globalization. Third molar agenesis exemplifies how human traits emerge from a dynamic relationship between evolutionary constraint and phenotypic plasticity, capturing both historical trends and recent selective or non-selective pressures. By framing agenesis within this broader evolutionary context, this meta-analysis highlights the limitations of single-cause explanations and underscores the need for integrative, multilevel approaches.
